If you will be undergoing active treatment at the Texas Medical Center for leukemia, lymphoma, or myeloma, and you live more than 50 miles from the Texas Medical Center, or if you are a stem cell transplant patient living more than 25 miles from the TMC, you are eligible to apply.

If you download and print the form, please complete all information, and and fax it to us at 713-391-8365. Please provide an email address so that we many confirm receipt of your application. Apartments will be filled based on earliest application date matched with ”need date”.
Once you have submitted your application, please email us at mail@halohousefoundation.org every 30 days to let us know if your “need date” has changed, and if you still need an apartment and wish to remain on the Halo House Wait List.
